Your Social Security Number will be used to run a credit check associated with
your credit card.
To verify your
debit/credit card, a temporary $10 hold also will be placed on the card, which
most banks will remove within 3-4 business days.

By providing a credit/debit card to qualify, you authorize DISH Network to
automatically charge or debit your card for any unreturned equipment fees and
cancellation fees if applicable.

The name associated with the major credit or debit card used to credit qualify
the account must match the name of the customer listed on the DISH Network
Account.

You will see a $99.00 activation fee on your first bill unless you have agreed
to the 24-month commitment.

All payments for programming are non-refundable and pricing and programming are
subject to change.

There will be a $5.00 fee charged anytime you downgrade programming. For adult
programming, this fee is $10.00.
First Bill, Monthly Rate & Fees

By purchasing you have authorized DISH to charge your debit/credit card the
applicable amount at time of sale, which is payment for your first-month of DISH
service and all one-time fees. This allows DISH to start your programming
immediately after installation.

The applicable amount is fully refundable if DISH is not successfully installed
at your home or you cancel prior to activation. Most banks will issue the
refund within 3-4 business days.

Payment includes:
- The first month of service
which includes programming and introductory credits, and [DVR service and
additional receiver fees if applicable]
- Activation fee [if
applicable],
- Equipment upgrade fee
[if applicable],
- HD Programming Upgrade of
$99 for "HD For Life" [if applicable],
- Dish Promotions will collect the payment for your first
month of DISH service, which starts once your installation is complete. Your
payment for your second month of service will not be due for another 35 days.

You will receive your second month bill within 3 weeks.

Your bill will be reduced by $15 per month for 12 months. If Latino package your
bill will be reduced by $10 per month for 12 months.
o To receive these credits, you
must maintain a minimum of America's Top 120 or Dish LATINO Clasico level
programming.
o After 12 months, your programming
will be billed at normal rates.
Service Plan
- You will receive the
Service Plan free for 6 months, which includes free shipping on equipment
replacement, and reduces the cost of in-home service from $95.00 to $15.00.
- If you do not cancel the
Service Plan at the end of the 6-month promotional period, you will be billed at
the normal rate of $6.00 per month.
Cancellation Fee
- In order to get all these
great savings, you need to agree to a 24-month commitment. If you cancel
early, you will be charged a cancellation fee of $17.50 for each month
remaining. In addition, this fee may be charged to the card used to
qualify.
Equipment Fee
- Because all equipment is
leased to you, BUT owned by DISH, it must be returned to us within a minimum of
30 days of cancellation or you will be charged an equipment fee ranging from
$100 to $400 per receiver. This fee may be charged to the card used to
qualify.
- If you don't pay your bill
and your account is disconnected, the equipment fee will be charged immediately.
- Any equipment fees will be
refunded if you return your equipment.
- Should you discontinue your
services or do not maintain minimum programming, boxes will be shipped to the
address on your account so you can return the equipment, which includes your
receivers, smart card, remote controls, and LNBF's.
HD for Life [if applicable]
- You also will receive "HD
For Life," which means that the normal $10 monthly HD add-on fee (which permits
a DISH subscriber to receive an HD simulcast of standard-definition programming
in qualified packages) will be waived until cancellation or disconnection of
your DISH service for any reason.
o In order to get HD for Life, you
agree to a 24-month commitment and you agree to maintain Auto Pay with Paperless
Billing.
o If Auto Pay with Paperless
Billing is removed from your account, you may be charged the full price for HD
programming.
HBO/Showtime [if applicable]
- You will also receive
HBO/Showtime free for 3 months.
o To receive these credits, you
must agree to Auto Pay with Paperless Billing.
o If you do not cancel HBO/Showtime
at the end of the 3-month promotional period, you will be billed the normal rate
of $24.00 per month.
o If Auto Pay with Paperless
Billing is removed from your account, you may be charged the full price for
HBO/Showtime.
Auto Pay [if applicable]
- DISH will automatically
charge your credit or debit card on the monthly due date for all charges
incurred, including monthly programming fees and any other charges that you
might incur such as pay-per-view fees, early cancellation/termination fees,
equipment fees, and other similar non-recurring charges. You agree to contact
DISH at least 7 days prior to the due date to make any changes to your auto-pay
status.

DIGITAL HOME ADVANTAGE TERMS AND CONDITIONS CREDIT FOR 12
MONTHS. Requires 24-month commitment to Digital Home Advantage. A customer who
subscribes to qualifying programming will receive a credit for each of the first
12 months as follows: $15 per month with America's Top 120, America's Top 120
Plus, America's Top 200 or America's Top 250; $10 per month with DishLATINO
Clasico, DishLATINO Dos or DishLATINO Max. Customer must maintain qualifying
programming to receive all credits. Customer will forfeit credit in the case of
a downgrade from qualifying programming or service disconnection prior to 12
months of credits. Maximum number of months with credits is 12.
3 FREE MONTHS OF HBO AND SHOWTIME PACKAGES. Requires
continuous enrollment in AutoPay with Paperless Billing. A customer who
subscribes to qualifying programming (a minimum of DISH America, DishMEXICO or a
qualifying International package) plus HBO and Showtime will receive a credit
equal to the monthly cost of the movie packages for each of the first 3 months.
After 3 months, movie package programming will continue to be billed at
then-current price unless customer elects to downgrade. Value of offer is $72.
If customer downgrades within 30 days of end of 3-month period there will not be
a Change of Service Fee for this downgrade.
DIGITAL HOME ADVANTAGE. ALL EQUIPMENT REMAINS THE PROPERTY
OF DISH NETWORK AT ALL TIMES AND MUST BE RETURNED TO DISH NETWORK WITHIN THIRTY
DAYS OF ACCOUNT DEACTIVATION OR CUSTOMER MUST PAY AN UNRETURNED EQUIPMENT FEE
RANGING FROM $100 TO $400 PER RECEIVER. Customer pays a $99 Activation Fee. The
$99 Activation Fee will be waived for a customer who agrees to a 24-month
agreement. If qualifying service is terminated prior to end of 24 months, a
cancellation fee equal to the lesser of $420 or $17.50 per month for each
cancelled month of service will apply. Customer will be required to pay a lease
upgrade for select receivers, based on model and number of receivers. The lease
upgrade fees are not deposits and are non-refundable. Offer requires qualifying
programming purchase. Customer must provide Social Security Number (or the last
4 digits of the Social Security Number) and a valid major credit card.
Participation is subject to credit approval. Customer will be charged a monthly
equipment rental fee for each receiver beyond the first, based on model of
receiver. Participating retailers may require additional terms and conditions.
The first month of DISH Network service must be paid at time of activation. With
24-month agreement, customer will receive the Service Plan at no cost for the
first 6 months. At the end of the 6-month period, customer must call DISH
Network to cancel Service Plan or a $6.00 monthly charge will apply for the
service. There will not be a Change of Service Fee for this downgrade.
Digital Home Advantage offer is available from DISH
Network and participating retailers for new, first-time DISH Network residential
customers (only 1 per household) in the continental United States through
9/28/10. All prices, packages and programming subject to change without notice.
Number of channels may decline. Local and state sales taxes may apply. Where
applicable, monthly equipment rental fees and programming are taxed separately.
Activation Fee is completely non-refundable once the system is activated. The
Digital Home Advantage includes a satellite TV system with an eligible
configuration of up to 6 tuners, Standard Professional Installation and choice
of qualifying programming. For this offer, Standard Professional Installation
includes typical installation of one single-dish antenna configuration, typical
hook-up of up to 6 tuners to up to 6 TVs and equipment testing. More complex
installations may require additional fees; other installation restrictions
apply. Prices valid at time of activation only; additional fees will apply to
upgrade after installation. No more than 6 leased tuners per account are
permitted. No more than 3 leased receivers per account are permitted. Any
unreturned equipment fees will automatically be charged to the customer's DISH
Network account or the customer's credit or debit card provided to DISH Network.
DISH Network shall determine eligibility for this offer in its sole and absolute
discretion. Programming and other services provided are subject to the terms and
conditions of the promotional agreement.
